        Have you ever had an allergic reaction to Mometasone Furoate, Beconase (Beclometasone) or Benzalkonium Chloride?

                You may not have hayfever (allergic conjunctivitis) if you have symptoms in only one eye. We recommend you consult your doctor or optometrist for further advice.

                • Glaucoma/raised intraocular pressure
                • Recurrent or untreated nasal infections
                • Unresolved complications following nasal surgery

                  Are you currently taking any medication (including over the counter, prescription or recreational drugs)?
                  Are you taking any of the following medications?
                  • Warfarin, Rivaroxaban, Dabigatran, Apixaban (or any other anticoagulants)
                  • Daily NSAIDS (e.g. Naproxen, Diclofenac or Ibuprofen)
                  • Daily Aspirin
                  • Anti-Epileptics
                  • HIV retroviral treatments
                  • Systemic steroid treatments
                  • Immunosuppresive treatments
                  • Previous hypersensitivity to steroid treatment (e.g. topical steroids for eczema)

                  Are you taking any oral medications for allergies/hayfever?
                  You should only take one antihistamine tablet a day. Do not mix oral antihistamines. Stick to one anthistamine and take once daily only.

                  We will be able to prescribe loratadine (clarityn) or cetirizine (piriteze), but if you have not taken antihistamines before, we cannot prescribe fexofenadine (telfast).

                  • You will contact us and inform your GP of your medication if you experience any side effects of treatment, you start new medication and/or your medical conditions change during treatment

                  • You understand our prescribers take your answers in good faith and base their prescribing decisions accordingly, and that incorrect information can be hazardous to your health

                            Do you understand that you should not use nasal steroids for more than 3 months each year unless advised otherwise by your GP?
                              Although very little of the medication makes it into the body tissues, prolonged and sustained use can lead to systemic side effects from steroids, as well as risking interaction with other medications. If you intend to use the medication for more than 3 months of the year please consult your GP.

                              Do you understand there is a possibility that oral antihistamines may cause drowsiness and affect driving or operating machinery? You should check that the antihistamine tablets you receive do not make you feel sleepy or dizzy before driving or operating machinery.
